Healthy Digestion Begins with Healthy Teeth

You pay attention to what you eat. After all, your oral health depends on it. Without the necessary proteins, vitamins, and minerals in your diet, your teeth and gums will suffer. But did you know it’s a two-way street? Without healthy teeth and a healthy bite, your digestive system can suffer as well.

Because digestion doesn’t start in your stomach—it starts in your mouth! Let’s take a quick look at how the digestive system operates.

  • Teeth

The first step in digesting is breaking down foods so your body can extract their nutrients more easily. Healthy teeth are essential here. Incisors and canines tear food into smaller pieces while molars grind these pieces into an easy-to-swallow, paste-like consistency. While we chew, the surface area of the food increases, allowing the digestive enzymes in saliva, the stomach, and the small intestine to work more efficiently.

  • Salivary Glands

Saliva production increases when we eat. Digestive enzymes in saliva begin breaking down carbs into sugars, and separating fat molecules. Saliva also contains antimicrobial peptides which are important for oral and gut health.  

  • Esophagus

This muscular tube connects the back of the throat to the stomach. As we swallow, muscles in the esophagus contract and relax, an involuntary movement called peristalsis. These contractions push food down into the stomach. Peristalsis also occurs in the stomach and intestines, efficiently moving food through the rest of the digestive system. 

  • Stomach

Inside the stomach are gastric acids and enzymes which break food down further to help the small intestine make use of these nutrients in the next stage of digestion.

  • Small Intestine

This organ works to reap the benefits of our healthy diets. The small intestine absorbs about 95% of the nutrients from our digested food—carbohydrates, proteins, fat, vitamins, and minerals—and transfers these nutrients to the circulatory system to be used throughout the body. Any remaining food particles travel to the colon, or large intestine.

  • Large Intestine

Bacteria in the large intestine help process soluble fiber, which we can’t digest on our own. The large intestine is also where insoluble fiber and anything else undigested are converted to solid waste. 

Now, let’s look at how a malocclusion, or bad bite, can disrupt the digestive process.

In some cases, malocclusions are mild, and require little or no treatment. A more serious malocclusion, though, can impact the way the jaws fit together, making it difficult or impossible for the jaws and teeth to align correctly for efficient and comfortable chewing. What are some possible consequences?

  • Uneven bite pressure

With a healthy bite, your teeth and jaws fit together properly to let you get the most out of your meal. When teeth and jaws don’t meet properly, it’s hard to chew food into the paste-like consistency necessary for rest of our system to process it smoothly.

  • Reduction in digestive enzymes and peptides

Difficulty chewing means less chewing. Less chewing means less saliva, and your system won’t benefit from the digestive prep work and gut protection which saliva typically provides. 

  • Uncomfortable swallowing

Insufficiently chewed food makes peristalsis in the throat and other digestive organs more difficult. Larger pieces of food can become stuck in the esophagus, causing painful swallowing or even choking.

  • Higher risk of stomach problems

When food isn’t chewed thoroughly, the stomach has to work harder, and the risk of gastroesophageal reflux increases. Highly acidic gastric juices can back up into the esophagus and mouth, causing heartburn, chest pain, sore throat, and vomiting. 

  • Bowel irritation

When food particles are too large, the small intestine must work harder to break them down and to absorb their nutrients. Insufficiently digested food can also upset the bacterial balance in the large intestine. These problems can cause indigestion, constipation, gas, and bloating.

  • Changes in nutrition 

Orthodontic problems can also lead to nutritional imbalances even before we start to digest. Soft foods and liquids are often chosen over proteins, fruits, and vegetables when chewing pain is a regular occurrence. While a soft diet is fine for a few days if your mouth is a bit sore following an orthodontic or dental treatment, it can be difficult to get the all the nutrients your body needs when you only eat soft foods.

The Herbst® Appliance

Maybe you’ve known people with braces and aligners—maybe you’ve worn them yourself!—so if braces or aligners are in your teen’s future, you have some idea what to expect and when to expect it.

But quite often, orthodontic issues require more treatment than braces alone can provide. When misalignment affects not only the teeth but the jaw as well, treatment can be more effective when it begins earlier and makes use of a different kind of appliance—the “functional appliance.”

During the years your child’s bones are still rapidly growing and forming, around the ages of eight to 14, functional appliances can help guide tooth movement and encourage jaw growth and development. One of the most widely used of these devices is the Herbst® appliance.

What does the Herbst appliance do?

There are several types of malocclusions, or “bad bites” treated by Dr. Gina Pinamonti. A common condition called a Class II malocclusion occurs when the upper jaw and teeth project too far forward over the lower jaw and teeth. Signs of a Class II malocclusion might include an overjet (protruding upper teeth), and/or a small or recessive lower jaw.

While correcting this malocclusion often enhances facial symmetry, which can be very important for a child’s confidence, correcting a Class II malocclusion also promotes jaw and dental health. Misaligned teeth are more difficult to clean, which can lead to decay and gum disease. Bite problems can cause persistent jaw pain and damage to the teeth. And, with an overjet, a child’s upper teeth are more at risk for injury.

The Herbst appliance was developed to treat this kind of malocclusion. It moves the lower jaw and teeth forward to create a balanced, healthy smile.

How does the Herbst appliance work?

The Herbst appliance is fixed in place with stainless steel bands or crowns that are secured to four teeth in the rear of the mouth, often the first molars on each side of the upper and lower jaws. The band or crown on each lower tooth is equipped with a small bar that extends toward the front of the mouth.

An arm on each side links the bands or crowns on the upper teeth to the front of the bar assemblies on the lower teeth. Each arm consists of a rod that fits smoothly into a tube. The telescoping action of the rod and tube allows the mouth to open and close normally. When the mouth is closed, the arms on both sides telescope shut, forming compact cylinders that hold the jaw forward.

While a child’s bones are still growing, the lower jaw’s new forward position can stimulate further bone growth and remodeling to maintain the jaw in that forward position. The Herbst appliance also has a restraining effect on the forward movement of the upper jaw. The result is a steady, noticeable improvement in the relationship between the upper teeth and jaw and the lower teeth and jaw.

Is the Herbst appliance hard to take care of?

The Herbst appliance is fairly low maintenance, but, like any orthodontic gear, it should be treated with care.

  • It’s important to watch your child’s diet, because sticky, crunchy, and chewy foods can damage the appliance. Save the caramels for a post-treatment celebration!
  • Carefully cleaning around the appliance is necessary, because a buildup of bacteria and plaque leads to consequences like bad breath, gum disease, and tooth decay. A water flosser can make reaching and cleaning tight spots easier if a brush alone isn’t effective.
  • If the Herbst appliance is damaged, some minor fixes might be doable at home with instructions from your treatment team. But if a band or crown comes loose, or if there’s a problem you’re unfamiliar with, call our Pittsburg, KS office right away.
  • To help avoid the need for minor (or major) fixes, playing with the appliance with fingers or tongue, nibbling on pens and pencils, chewing on ice, or any other risky habits should be strictly off limits.

Overbite Overview

An overbite is one of the most common malocclusions. If Dr. Gina Pinamonti and our team have diagnosed you with an overbite, you probably have lots of questions. Let’s try to answer some of them!

Just what is an “overbite”?

A malocclusion is another way of saying that you have a problem with your bite, which is the way your jaws and teeth fit together when you bite down. In a healthy bite, the front top teeth project slightly beyond, and slightly overlap, the bottom teeth. A normal overlap is generally considered one or two millimeters.

An overbite is a Class II malocclusion, and means that the upper front teeth cover more of the lower teeth than they should. But that’s a very general definition, and we will diagnose and treat your own, very specific, bite and teeth alignment.

Because overbites aren’t all alike. They might be barely noticeable. Upper teeth might overlap lowers by an extra millimeter or two. In more severe overbites, the upper teeth might cover the lower teeth completely. The amount of overlap and the cause of the overbite will determine your treatment.

What causes an overbite?

Overbites can be dental, caused by tooth alignment, or skeletal, caused by bone development, or a combination of both. They are usually hereditary, so, most often, an overbite is something you’re born with.

The size and position of your jaws, the shape and position of your teeth, all affect your bite alignment. But early oral habits, such as prolonged and vigorous thumb-sucking or pacifier use can contribute to overbite development. Missing teeth and bruxism, or tooth grinding, can also affect the alignment of your bite.

How do we treat an overbite?

There are many types of treatment available. Dr. Gina Pinamonti will recommend a treatment plan based on the type and severity of your overbite. Because some treatments are effective while bones are still growing, your age plays a part as well.

  • Braces and Aligners

If dental issues are the main reason for your overbite, braces or clear aligners can be very effective. Rubber bands are commonly used to help bring teeth and jaw into alignment.

  • Functional Appliances

If the overbite is caused by a problem with upper and lower jaw development, devices called functional appliances can be used to help guide the growth of the jawbones while a child’s bones are still forming.

For young patients, there are several appliances that can help correct an overbite. Some, like the Herbst appliance, work inside the mouth, while others, like headgear, are worn externally. Your orthodontist will recommend the most effective appliance for your needs.

  • Surgical treatment

In some cases, where the problem is skeletal rather than dental, surgical treatment might be necessary to reshape the jawbone itself. This is especially true for adults, whose bones have finished forming.

If we recommend surgery, oral and maxillofacial surgeons are experts in surgical procedures designed to create a healthy and symmetrical jaw alignment. Dr. Gina Pinamonti will work with your surgeon to design a treatment plan, which will usually include braces or other appliances following surgery.

Sometimes, a very slight overbite won’t require treatment. A serious, moderate, or even mild overbite, though, can lead to many dental and medical problems, including:
